Applicants must be U.S. citizens or authorized to attend school in the U.S. and enrolling in four-year college or university. Students must complete the application and provide school acceptance letter along with an essay describing their community service.

Applicants must be U.S. citizens and graduating high school seniors or currently enrolled undergraduates in a four-year college institution whose permanent residence is located in one of the following Indiana counties: Jefferson County, Kentucky and Clark, Floyd, Harison, Washington, Scott or Jefferson County.

Applicants must be military veterans of the U.S. Armed Forces or immediate family members enrolling in a trade school, vocational program or an accredited institution earning credits toward an associate, baccalaureate or graduate degree. Students must write an essay pertaining to the given topic about prescription drugs.
