Applicant must reside or be employed in Lorain County desiring aid for initial and advanced medical education or be pursuing education and training in all fields of medicine. FAFSA, Student Aid Report, and tax forms are required.

Applicant must have a minimum 22 ACT score (1050 SAT, critical reading and math only) and a minimum 3.0 GPA. Applicant must major in athletic training, nursing, or any pre-health profession program. Leadership recommendation is required.

Applicant must be interested in the pre-medical, pre-dental, physician assistant or physical therapy programs. Minimum 3.8 GPA and 1300 SAT score required.

Applicant must demonstrate financial need and plan to pursue a career in the medical profession.

Applicant must plan to attend medical, dental, podiatry, veterinary, or optometry school after graduation.

Applicant must be a minority student majoring in pre-med.

Applicant must be a pre-med graduate of Adams State College and have been accepted by an accredited institution pursuing the degree of doctor of medicine.

Applicant must be a resident of Johnson County pursuing advance education or certification in the long-term healthcare field.

Applicant must be a graduating senior from Glendale Unified School District who is accepted and will attend an accredited college or university to pursue a medical or pre-med program.

Applicant must be a graduate from a Greene County, AR high school who is majoring in pre-medicine or attending medical school.

Applicant must be a student organizer for the National Cesar E. Chavez Blood Drive Challenge. Selection also based upon cumulative GPA, leadership, and community service.

Applicants must be African American students enrolled at an accredited medical school in the New York City area. Students must demonstrate leadership ability and commitment to serve.

Applicants must be members of underrepresented minority groups or from socioeconomically disadvantaged backgrounds. Students must be currently enrolled in medical school or a graduate-level nursing or physician assistant program.
